This Ordinary Membership Application form is suitable for individuals and households where there are multiple occupants. Where you have chosen to join more than one individual in a household as an ordinary member you will choose one person to be the nominated member and add the other individual’s names where applicable in the form.

The ordinary membership fee is $20 per year.

When paying by credit card please tick the 'Credit Card', box and complete your credit card details at the bottom of this form and then submit your form. Your payment will be acknowledged by Eway, our card facility system.

When paying by direct deposit, please tick 'Direct Deposit' box and use your name as a reference on your payment. After making your payment send a copy of your deposit receipt to membership@qmhoa.org.au or upload it in the upload section at the bottom of this page. Then submit this form. We will then marry up your payment and your submitted application form.

When paying by cheque, please tick the 'Cheque' box, write your name on the back of the cheque, and post your cheque to:
PO Box 932 Booval Fair Qld 4304.
Your cheque will be matched to your completed form and we will send you a receipt.
